What is the best method / technique/way to provide incentive and the best way to measure it effeciently ?

I have found measuring the way an incentive is given a rather difficult exercise as we end up making different charts for various positions. I also feel one extra staff is required just to check the key measurable areas of a particular staff on a daily basis and then to combine it monthly to provide incentives
What is the way out to have a great incetive program for all staff , which is easily measurable and make sure that it is followed up as an integral part of the job responsibility.
Also is it fair to have a combined "Employee of the month" programme as invariably the back of the house employees hardly have a chance to win it and sometimes its more out of sympath you end up giving them an award. So how do we make this a fair practice
